Seems simple and light weight. Pretty complete. Fully customizable. Have not updated since June 2003, not too many support options.
(Though simple enough to figure it out)

Advanced Poll
System requirement: PHP and MySQL (supports text-based DB, too)
It spits out a PHP code and you have to cut and paste them, but template is configurable and admin is powerful.
Lots of room for customization but lots of room for improvement, more step to create new poll
(it’s not fully automated) But you can cut and past the code into any page …more flexibility.
